Field Trip Request Form

The Plants for Human Health Institute has a dedicated training lab for STEM enhancement opportunities. The lab is coordinated by Amy Bowman, STEM Education Extension associate. Please submit this form to schedule a field trip for your class/organization. All activities are led by Ms. Bowman and must be scheduled for a time frame within the work week (Monday-Friday, 8am-5pm). Due to space limitations, the participant maximum is 30 students. Prior to the field trip, each student/guardian must complete a publicity release and liability waiver.
